Our clinic is currently accepting new patients and will make every effort to accommodate your schedule requests. Typically, Tuesdays and Thursdays and reserved for seeing patients in clinic, while Mondays are surgical days as needed. Although we recognize that not everyone's schedule fits these parameters, and injuries tend to occur at inevitably inconvenient times, we will try to be flexible in making time available to see patients.
Our goal is to provide all the necessary diagnostic, imaging, treatment, and therapy plans for you during your visit, so that the path to healing begins as soon as possible. In order to accomplish this, we request of you a few things:
First, please arrive to your initial appointment 15 minutes early to ensure your Patient Information Forms are completed.
Secondly, it is absolutely essential for our clinic to have as much information as possible, so if you have had prior doctor visits, surgeries or imaging, please obtain any office reports, surgical reports and pictures, and all x-ray or MRI films, including the x-ray and MRI reports. Please contact the appropriate clinic or hospital for these items. You may choose to personally bring them with you to your appointment or have them sent to our office:

Records may also be faxed to our number at 612.332.1019. If this information cannot be obtained by the time of your scheduled appointment, please call our office to reschedule.
And finally, we need you to bring your insurance card or workers' compensation information. The last thing we want is for treatment to be unnecessarily delayed because of inadequate insurance information.
